EHS transitions Student-Visitor Incident Report form online

In order to streamline processes and provide a more accessible system, the WVU Environmental Health and Safety (EHS) team has transitioned its Student-Visitor Incident Report form online.
Students or visitors who experience an injury or incident on WVU property should submit a report online through the form. Previously, students and visitors completed a fillable PDF or printed form and submitted it by email to EHS.
It is critical that students and visitors submit a report within 24 hours of the injury/incident.
The new reporting system is accessible by phone or computer and allows reports to be submitted directly to both EHS and Risk Management.
The move to a digital reporting form is intended to improve and streamline current processes, making it easier to use and encouraging earlier reporting of incidents. This change will help ensure faster response times and better tracking of incidents.
As a reminder, the Employee Injury/Incident Report is also available online for employees to report an injury/incident within 24 hours.
